Indication of Minirin:

Minirin is used in the treatment of cranial diabetes insipidus, a condition which results in excessive urination. It is also useful in treating bed wetting (nocturnal enuresis) problems in children over the age of 6, where the use of bed wetting alarms is not possible or ineffective. This drug is also used in diagnosing the cause of disease in people with excessive urination. Specific bleeding disorders can also be treated with this medication.

Action of Minirin:

The active component of Minirin is desmopressin, which is a chemical related to a natural hormone called antidiuretic hormone (ADH) or vasopressin. This is produced by the pituitary gland of the body. ADH has two main effects on the body. Firstly, it acts on particular receptors in the kidneys causing them to retain water in the body, by preventing excessive loss of water in the urine. Secondly, ADH causes narrowing of blood vessels (vasoconstriction), thereby limiting blood flow to a particular area of the body. In diabetes insipidus, there is a reduction in the function of ADH resulting in more water being lost from the body through urine. This results in the production of large amounts of dilute urine. Desmopressin is used in the treatment of this disorder as it acts on the ADH receptors in the kidneys, mimics the effects of ADH, and therefore prevents dehydration caused by excessive loss of water. As desmopressin reduces the production of large amounts of urine, it is also useful in treating bedwetting and the passage of urine at night. Desmopressin is also used to prevent bleeding in blood clotting disorders such as haemophilia, as it increases substances in the blood that are responsible for blood clotting.

Dose Advice of Minirin:

The dose of the drug differs from person to person and the doctor determines the dose by starting low and slowly increasing it until a suitable dose is found.

Minirin Tablets:

Cranial diabetes insipidus:
  • The starting dose in both adults and children is 100 micrograms taken three times daily.
  • The total daily dose usually lies somewhere between 200 and 1200 micrograms a day.

    Bedwetting:
    The initial dose should be 200 micrograms taken before bed. If this isn't effective it can be increased to 400 micrograms. The doctor may ask their patient to limit their intake of fluids, since there is a risk of having too much fluid in your body when taking this drug.

    Minirin tablets are intended to be used for a period of 3 months; once this time limit has been reached the patient should see their doctor for a treatment review.

    Nasal Solution:

  • This is administered via a dropping bottle.

    Diabetes Insipidus:
  • Adults 10-40micrograms/day
  • Children 2.5-20 micrograms/day given in two doses

    Diagnosis of renal concentrating capacity:
  • Adults ≤ 40micrograms
  • Children ≤ 20micrograms
  • Infants ≤ 10micrograms

    Minirin Injection:

    Diabetes Insipidus
  • Adults 1-4 micrograms/day
  • Children ≤ 400 nanograms/day given via IVI or IMI in 2 divided doses

    Diagnosis of renal concentrating capacity:
  • Single doses of Minirin are used for this purpose.
  • Adults ≤ 4mcg IMI
  • Use of this form of Minirin is not advised in children

    Bleeding disorders:
  • Administered via an IV infusion.

    Nasal Spray:
  • Given into the nostrils using a spray pump and the dose is divided between the nostrils. It is important not to inhale whilst using the spray pump.

    Bedwetting:
    Children > 6 years are initially started on 20 micrograms before bed, and the dose being adjusted according to the response to the treatment. The normal dosing range lies between 10-40 micrograms.

    Cranial diabetes insipidus:
  • Adults: 10 - 40 micrograms/day
  • Children: 2.5-20 micrograms/day, given in 2 divided doses Diagnosis of renal concentrating capacity:

  • Restrict fluid intake especially in children.
  • Adults: ≤ 40micrograms
  • Children: ≤ 20micrograms
  • Infants: ≤ 10micrograms
